Ten Aboriginal Community Controlled Organisations and four Community Service Organisations have been commissioned to deliver Home Stretch WA to eligible care leavers in metropolitan Perth and all regional areas of Western Australia.
For young people living outside of provider’s approximate areas of service delivery, the closest Home Stretch WA providers will discuss the referral and determine whether they will share the support of a young person, depending on the young person’s Country of origin, their preferences for a specific service and the ability of the provider to offer face-to-face support, where practical.
Young people who are regularly moving between regions, may also be referred to multiple providers, to support their needs where they are living.
