Each practice area is interdependent, and together they are designed to be delivered in an integrated and seamless way. Home Stretch WA is designed to be adaptable to local communities and regional areas to meet the specific needs of each context.\u00a0<\/span><span style=\"font-size: 1rem;\">Every young person will receive an individualised service based on their needs and aspirations, offered through a persistent, consistent, hope inducing relationship with their Transition Coach.\u00a0<\/span><span style=\"color: var( --e-global-color-text ); font-family: var( --e-global-typography-text-font-family ), Sans-serif; font-weight: var( --e-global-typography-text-font-weight ); font-size: 1rem;\">Whilst the Home Stretch WA Model allows for flexibility, the following are to be adhered to;<\/span><\/p><ul><li>Department of Communities Policies, Guidelines &amp; Forms<\/li><li>Alignment with <a href=\"https:\/\/homestretchwa.com\/cop\/wp-content\/uploads\/2025\/04\/TC-PT-1.pdf\">Transition Coach Practice Principles<\/a><\/li><li>Practice consistent with standards outlined in the <a href=\"https:\/\/homestretchwa.com\/cop\/wp-content\/uploads\/2025\/08\/YOUNG-PEOPLE-RIGHTS.pdf\">Standards of Support for Young People\u00a0<\/a> \u00a0&amp; <a href=\"https:\/\/homestretchwa.com\/cop\/wp-content\/uploads\/2025\/04\/OP-PG-013-Home-Stretch-WA-Model-Integrity-Framework-V.2_March_2025-1-1.pdf\">Home Stretch WA Model Integrity Framework\u00a0<\/a><\/li><li>Commitment to Aboriginal Ways of Working within the Home Stretch WA Service Model<\/li><\/ul><div>\u00a0<\/div><p>Each practice area has its own practice guidelines &amp; resources to support Home Stretch WA Service Providers to deliver the service.\u00a0<span style=\"color: var( --e-global-color-text ); font-family: var( --e-global-typography-text-font-family ), Sans-serif; font-size: 1rem; font-weight: var( --e-global-typography-text-font-weight );\">Providers are encouraged to update practice resources and communication materials to suit their local contexts.\u00a0<\/span><span style=\"font-size: 1rem;\">\u00a0<\/span><\/p><p>CoP members can access online training via the training login for a full induction into the Home Stretch WA Model.<\/p><p>As of July 2025 this knowledge hub is no longer updated and serves an online archive of practice resources for Service Providers.<\/p><p><span style=\"color: var( --e-global-color-text );
